I hereby authorize Friends of Public Radio Inc. to initiate debit entries to my account indicated on the electronic form. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.
This authorization is to remain in full force and effect until Friends of Public Radio Inc. has received written notification from me of its termination in such time and in such manner as to afford Friends of Public Radio Inc. a reasonable opportunity to act on it.
Furthermore, if any such electronic debit(s) should be returned by my financial institution as Non-Sufficient Funds ( NSF ), I authorize Friends of Public Radio Inc. to collect a returned item fee up to the amount required to process the return item by electronic debit from my account identified above.
The recurring EFT payment will be withdrawn from my bank account and will be updated there on within one business day of the last day of each month.
I understand and authorize all of the above as evidenced by my signature on the appropriate electronic form.
